Biostimulators are prescribed to a face, not sold by the vial. What is chosen and how much depends on where collagen has been lost, on skin thickness and on how much laxity there is — and on what you want to change, which is not always what is most visible in a mirror.

The consultation covers skin quality and thickness, the pattern of volume loss, previous treatments and any fillers already in place, planned events with dates, and your medical history. Photographs in standard lighting are taken so that change can be compared objectively later — with a gradual effect, memory is an unreliable measure.

A course is typically two to three sessions with intervals of four to six weeks, then reassessment. The interval is not a scheduling convenience: the tissue response develops between sessions, and treating again too early means adding material before you can see what the previous session did. Where the concern is loss of volume that needs immediate correction rather than gradual firming, a hyaluronic acid filler may be more appropriate, and the two are sometimes combined in a plan rather than chosen against each other.

Maintenance follows the same logic: ageing continues, so a plan is reviewed rather than repeated automatically. Individual results vary.

Expect swelling, tenderness and sometimes bruising at the injection points for a few days. With poly-L-lactic acid there is often a temporary volume effect on the day from the fluid the preparation is reconstituted in; it settles within days, well before the actual collagen response begins.

Massage matters. With PLLA in particular, the aftercare protocol of massaging the treated areas for several days is part of the treatment, not an optional extra: it distributes the particles and reduces the risk of nodules. Your doctor will show you the technique and the schedule.

Avoid strenuous exercise, saunas, steam and alcohol for the first 24 to 48 hours, and keep the treated skin protected from sun.

Risks. Swelling, bruising and tenderness are common and temporary. The characteristic risk of biostimulators is late nodules or palpable irregularity, which depends on injection depth, technique and aftercare; unlike hyaluronic acid, these cannot be dissolved, which is why preparation choice and placement are conservative. Infection is uncommon. Vascular complications are rare but serious.

Contact the clinic if you develop increasing pain, spreading redness, fever, a hard lump appearing weeks or months later, or any change in vision or skin colour after treatment. These are assessed promptly rather than watched.
