Wisdom Teeth Removal in Dubai

A wisdom tooth that has erupted into a functional position, bites against an opposing tooth and can be reached with a brush is left alone and watched. Removal is recommended for specific findings, not on principle:

Pericoronitis — recurrent infection of the gum flap over a partly erupted tooth, with swelling, bad taste and difficulty opening the mouth. One episode may be treated and observed; repeated episodes are an indication.

Decay that cannot be restored in the wisdom tooth itself, or, more importantly, on the back surface of the second molar where a tilted wisdom tooth traps plaque. That second molar is a tooth worth protecting — see fillings.

Cysts and resorption — a follicle around an unerupted tooth that has enlarged, or a wisdom tooth eating into the root of its neighbour on the scan.

Orthodontic and prosthetic planning, where the tooth interferes with treatment or with a planned restoration.

The single question that decides how a lower wisdom tooth is removed is where its roots sit in relation to the inferior alveolar nerve, the nerve that supplies sensation to the lip and chin. On a flat panoramic film the roots and the nerve canal are projected onto the same plane, and an overlap on the image tells you nothing about whether they actually touch.

A cone beam scan on our Planmeca Viso G5 answers it in three dimensions, with the field of view limited to the region in question rather than the whole jaw. Where the roots wrap around the canal, the plan changes: the tooth is sectioned and removed in pieces rather than levered out whole, or a coronectomy — removing the crown and deliberately leaving the root tips undisturbed — becomes the safer option. That decision is made before the appointment, not during it.

Upper wisdom teeth and erupted lower ones are usually straightforward under local anaesthetic: 20 to 40 minutes, and you drive yourself home.

Impacted teeth need a small incision in the gum, sometimes removal of a thin shell of bone, and sectioning of the tooth so it comes out through a smaller opening than the tooth itself. The socket is cleaned and stitched. Because ELEMENTAL is a licensed day surgical centre, the same procedure can be done under intravenous sedation or general anaesthesia with an anaesthetist present — the usual reasons being several impacted teeth in one session, a difficult airway or medical history, or anxiety that makes local anaesthetic unworkable. Sedation cases need fasting, a pre-operative assessment and someone to take you home; you are discharged the same day.

Which route suits you is decided at the consultation, after the scan, and quoted before you commit to anything.

The first 24 hours matter most. Bite on the gauze for an hour, keep an ice pack on the cheek in twenty-minute cycles, take the painkillers before the anaesthetic wears off rather than after, and do not rinse, spit or use a straw — the clot in the socket is what you are protecting. Cold, soft food; nothing hot.

From day two, warm salt-water rinses after meals, gentle brushing of the other teeth, and back to normal food as opening improves. Swelling peaks around the second or third day and then recedes; a stiff jaw and a bruise along the jawline are expected after a difficult lower tooth, not a complication. Most people are back at work after one or two days, longer after a deep impaction.
