All-on-4 Dental Implants in Dubai

This is the part that makes the concept work, and it is worth understanding before comparing prices.

Bone at the back of the jaw is the first to disappear after teeth are lost. In the upper jaw the sinus expands downward into the space; in the lower jaw the nerve canal limits how deep an implant can go. An upright implant placed at the back often has nothing to hold on to, which is why many full-arch plans need grafting.

Angling the rear implants forward lets them engage bone that is still dense — in front of the sinus in the upper jaw, in front of the nerve loop in the lower. The implant is longer, the contact with bone is greater, and the support reaches further back along the arch than its entry point suggests.

The practical consequence is that All-on-4 can often be done in a jaw where an upright six-implant plan would first require bone grafting and sinus lift. That is months of extra treatment avoided — when the anatomy allows it.

Four is a minimum, not a default. It works when the bone at the front of the jaw is adequate in height and density, when the bite forces are moderate, and when the opposing arch is not putting unusual load on the bridge.

Where it is worth considering more implants: a heavy bite, a history of grinding, a full natural or implant-supported opposing arch, or a long arch where the bridge would extend well behind the last support. In those situations All-on-6 spreads the load across more points and shortens the unsupported span at the back.

There is also the question of margin for error. With four implants, losing one is a serious problem for the whole restoration. With six, five can usually carry it while the situation is addressed. That difference matters more to some patients than to others, and it is discussed openly rather than glossed over.

Planning is done on a cone beam scan taken at the clinic on a Planmeca Viso G5. The scan gives bone height, width and density at each intended site, the position of the inferior alveolar nerve, and the outline of the maxillary sinus — which is precisely the information the angulation depends on.

The finished teeth are planned first and the implants second: where the teeth need to sit for your face, lip support and bite decides where the implants can go. Imaging is provided by our dental diagnostics department, and the bridge is made in the clinic's own digital dental laboratory.

Sometimes the scan says four implants are not appropriate. That is a useful answer, and it comes before treatment starts rather than during it.

Assessment. Examination, cone beam scan, review of medical history and medication, and a frank discussion of what the result can and cannot look like.

Surgery. Remaining teeth in the arch are removed, the bone is prepared and four implants are placed, the rear pair angled. Carried out in a licensed day-surgery theatre; the patient goes home the same day.

The temporary bridge. A provisional bridge is fitted so you are not left without teeth. Whether it is fitted the same day or after healing depends on how firmly the implants sit at placement — this is measured in theatre, not promised in advance. If stability is insufficient, the bridge waits.

Healing. Integration with bone takes a period usually measured in months and varies between patients. The temporary bridge is worn throughout and the diet is restricted.

The final bridge. Made, fitted, adjusted and reviewed. Material and tooth arrangement agreed beforehand.

Implant placement is surgery and it has risks, and your surgeon explains them to you in full at the consultation, before you give consent.

Smoking is a recognised risk factor for implant failure. Uncontrolled diabetes affects healing. Untreated gum disease, heavy grinding and some medications — including certain medications affecting bone — change the plan or rule the treatment out. All of these are reviewed at the consultation, and some patients are advised against implants.

Daily cleaning under and around the bridge, and professional review at set intervals, are part of the treatment rather than optional extras. Gum health is managed with our periodontology department.

Individual results vary. A full-arch bridge is not a lifetime device: it will need maintenance and, in time, repair or remake.

All-on-6. More supports, shorter unsupported span at the back, more margin if an implant is lost. Needs bone in six places, which sometimes means grafting.

Implant overdenture. Removable, clips onto two to four implants. Easier to clean, less expensive, and often the more durable choice where bone is limited or daily hygiene is difficult.

Conventional denture. No surgery at all. Less stable, particularly in the lower jaw, but a legitimate treatment.

Keeping the teeth you have. Where teeth are questionable rather than hopeless, periodontal treatment and conventional prosthetics may postpone or avoid a full-arch plan entirely.
