What a dental implant is

An implant replaces the root of a missing tooth. It is a titanium or titanium-alloy screw placed into the jawbone, which over a period of months bonds directly to the bone surrounding it. Onto that root are fitted two further parts: an abutment, which passes through the gum, and the visible restoration — a crown, a bridge or a denture attachment.

The reason to replace a root rather than simply fill the gap is that bone responds to load. A jaw that no longer carries a tooth resorbs, slowly and permanently, which is why a face changes shape after teeth are lost and why a denture that fitted five years ago no longer does. An implant transmits chewing load into the bone and gives it a reason to stay.

The practical consequence for the patient is that an implant stands on its own. It does not require the neighbouring teeth to be cut down to carry it, and it does not move when you eat.

The system we use. ELEMENTAL places Straumann implants, a Swiss-manufactured system with published long-term clinical data behind its surfaces and connections. The practical reason it matters to a patient is supply: prosthetic components stay available years after placement, so a crown that has to be remade a decade from now can still be built on the abutment already in your jaw. If your case calls for a different system, the dentist explains why before anything is agreed.

Planning: the part that decides the outcome

Implant treatment succeeds or fails at the planning stage. Before anything is placed, the clinic takes a cone beam CT scan on a Planmeca Viso G5 and reads the site in three dimensions: how much bone is present in height and width, how dense it is, where the inferior alveolar nerve runs in the lower jaw, where the floor of the maxillary sinus sits above the upper back teeth, and how the opposing teeth will meet the finished crown.

None of that is visible on a two-dimensional radiograph, and the specific risks of implant surgery — nerve injury, sinus perforation, an implant that emerges through the outer plate of bone — are the risks of placing without it. The scan is taken on site, so planning does not wait on an external imaging centre. What the scan shows and how it is used is described on the page about CBCT and dental X-ray.

Medical history is assessed at the same time: diabetes and its control, smoking, medication affecting bone metabolism or clotting, and the state of the gums, since gum disease must be stabilised before surgery.

Alongside the scan, both arches are recorded with a Dentsply Sirona Primescan AC intraoral scanner rather than a tray of impression material. The scan and the cone beam volume are matched in the planning software, so the position of the implant is chosen against the shape of the finished crown rather than the other way round.

The stages, and how long each takes

Placement is a surgical appointment under local anaesthetic, usually between forty-five and ninety minutes for a single implant. The gum is opened, the site is prepared to the planned depth and angle, the implant is placed, and the site is closed. Sedation is available where anxiety or the extent of surgery makes it appropriate.

Integration follows. The bone remodels around the implant surface over roughly three to four months in the lower jaw and three to six in the upper, longer where grafting was needed. During this period a temporary tooth can usually be provided so that a visible gap is not part of the treatment.

Restoration is the final stage: a scan of the implant position, design and manufacture of the abutment and crown, and fitting. Because the laboratory is in the clinic, the shade and shape are checked with the patient present and adjustments do not require a courier and a second week.

When bone has to be built first

Bone loss after tooth extraction is normal, and after several years the ridge is often too narrow or too short for an implant of adequate size. Where the deficit is small it is grafted at the same appointment as placement. Where it is larger, grafting is done as a separate procedure with a healing period of several months before the implant goes in.

In the upper back jaw the limiting structure is usually the maxillary sinus, whose floor descends as the molars are lost. Raising that floor and placing graft material beneath it — a sinus lift — creates the height an implant needs.

Whether grafting is required is a question the CBCT answers before treatment is quoted, not something discovered during surgery.

Replacing more than one tooth

Several missing teeth do not require an implant for each one. Two implants can carry a bridge spanning three or four units, and a full arch can be restored on a planned number of implants supporting a fixed bridge or a removable overdenture that clips onto them.

Which configuration is right depends on bone volume and distribution, on the opposing arch, on the bite and on what you want to be able to do with the result. A fixed full-arch restoration is not automatically the better answer: it is more demanding to clean, more expensive to remake and less forgiving of a mouth that is not maintained. An overdenture that clips onto implants is removable, easier to clean and often the more durable choice for a patient with limited bone.

The alternatives — a conventional bridge or denture — remain valid treatments and are discussed alongside, with their trade-offs stated rather than dismissed.

Safety and long-term care

Implant placement is surgery and it has risks, and your surgeon explains them to you in full at the consultation, before you give consent. Planning on three-dimensional imaging is what reduces most of them.

Late failure is usually biological rather than surgical. Peri-implantitis — inflammation and progressive bone loss around an integrated implant — is driven by plaque and behaves more aggressively than gum disease around a natural tooth, because an implant lacks the periodontal ligament and its blood supply. Smoking, uncontrolled diabetes, a history of periodontitis and irregular maintenance are the recognised risk factors.

Mechanical complications also occur: screws loosen, ceramic chips, and unmanaged night grinding shortens the life of the restoration. A night guard is part of the plan where bruxism is present.

The crown, made in our own laboratory

The implant is the part nobody sees; the crown is the part you live with. It has to match the neighbouring teeth in shade, translucency and surface texture, meet the opposing tooth correctly so the load is distributed, and emerge from the gum in a contour that can be cleaned.

The technician sees the case and the patient rather than a shipped model, shade is assessed in the same building, and adjustments do not depend on an external laboratory's schedule. The crown is designed from the Primescan file rather than from a plaster model poured off an impression, so what the technician works on is the same data the surgeon planned on, and a margin that did not read clearly is rescanned in seconds instead of at another appointment.

Maintenance after treatment

An implant does not decay, and for some patients that is misread as meaning it needs less attention than a natural tooth. The opposite is closer to the truth. The seal between gum and implant is weaker than around a tooth, so plaque left at that margin reaches bone faster.

A dental operating microscope is used at the restorative stage and at maintenance reviews. Residual cement left under the gum after an implant crown is cemented is a recognised trigger of peri-implant inflammation, and it is often invisible without magnification; the same magnification is what allows the implant surface and the soft tissue cuff to be inspected properly at review.

Maintenance means daily cleaning around the crown and between the teeth, and professional review at an interval set from your risk, commonly every three to six months. Professional cleaning around an implant is done differently from cleaning around a tooth: a steel scaler scratches the titanium neck, so deposits are removed with instruments that do not damage the surface and with air polishing using a fine erythritol or glycine powder — the technique commonly called Air Flow — which reaches into the sulcus around the crown. Reviews include probing around the implant and periodic radiographs to compare bone level over time, because early peri-implantitis is silent and is found on measurement rather than by symptom.

Cost

A single dental implant at ELEMENTAL costs from AED 5,000 per tooth (Osstem system); Straumann, Astra or Nobel implants are AED 6,000 per tooth, and a crown on the implant is AED 5,500. The CBCT scan, the surgical stage, any grafting and the final restoration are quoted separately in writing once the scan has been read.

The reason a single figure is not quoted before assessment is that the same missing tooth can mean an implant placed into adequate bone, an implant with simultaneous grafting, or a staged graft followed months later by placement — three different pieces of work. The written estimate lists each stage, so you can see what is included and compare it properly with any other quotation.
