Whitening is a treatment rather than a product, and it begins with an examination. The dentist establishes why the teeth are discoloured, because a single dark tooth usually means something different from generally yellow teeth and may point to a non-vital nerve rather than staining. Untreated decay, a leaking filling, gum inflammation and exposed root surfaces are all reasons to treat first, since gel reaching those areas causes pain and does not produce an even result.

A professional cleaning is normally done first. Removing surface deposits reveals the real starting shade, and some patients decide at that point that the cleaning alone was what they wanted. Stain from coffee, tea and shisha sits in the surface of the enamel and is lifted by air polishing — the jet of water, air and fine powder commonly called Air Flow — not by the whitening gel, which acts on the colour inside the tooth. The starting shade is recorded so the change can be judged against a record rather than a memory.

In-clinic whitening is done in a single appointment of about an hour and a half. The gums and lips are isolated with a protective barrier, a higher-concentration gel is applied to the teeth in timed cycles, and the shade is checked between cycles. The advantage is a visible change on the day and full control of the process by the clinician.

Take-home whitening uses thin custom trays made from a scan of your teeth and a lower-concentration gel worn for a set period each day over one to two weeks. It takes longer, it is generally gentler on sensitivity because the concentration is lower, and it leaves you with trays that can be reused for short top-ups later.

The two are often combined: an in-clinic session to make the initial change, then trays to finish and to maintain. Which route is chosen depends on your starting shade, your sensitivity, how even the discolouration is and how much time you have.

Temporary sensitivity is the most common effect of whitening, and it is expected rather than a complication. It usually appears during or shortly after treatment as short sharp twinges to cold or to air and settles within a day or two. Gum irritation can occur where gel contacts the tissue.

Sensitivity is reduced by treating its causes first, by applying desensitising agents before and during the course, by using a lower concentration over a longer period and by leaving days between sessions rather than pushing through. Tell the clinic if sensitivity is severe or persists beyond a few days, since that points to something other than the whitening itself — an exposed root surface, a crack, a leaking restoration or untreated decay.

Whitening is not carried out on children, during pregnancy or breastfeeding, or where there is active decay or untreated gum disease.

Whitening is not permanent. Colour returns gradually as pigments from food and drink re-accumulate, and how fast that happens depends far more on habit than on the whitening method. Coffee, tea, red wine, dark sauces, curry and tobacco all shorten the interval; so does a mouth that is not cleaned well, because plaque holds stain.

Most people hold a satisfactory shade for one to three years, and many maintain it with a short top-up in their existing trays once or twice a year. Regular hygiene appointments do more than any home product to keep the result, because surface stain is removed before it becomes established.

Whitening is the wrong tool for a tooth whose shape or position is the real problem, for teeth with visible enamel defects or old discoloured composite build-ups covering much of the front surface, and for severe intrinsic staining that will not lift to the shade you have in mind. In those situations the honest conversation is about veneers and smile design or about replacing the existing restorations, not about a stronger gel.

A single dark tooth is a different problem again and usually means the nerve has died. That tooth needs assessment and, where appropriate, root canal treatment before any internal whitening is considered.

Whichever route is taken, the sequence is the same: examine, treat what is unhealthy, whiten the natural teeth, then match any restorations to the final shade.
