The neck and décolleté often show tension and skin laxity earlier than the face. One reason is the platysma — a broad, thin muscle running from the collarbone to the jawline — which pulls downward over time and can create visible vertical bands.

Injecting a small dose into these bands can soften them and reduce the downward pull on the jawline. This is a technically demanding area: the muscle is thin and sits close to structures involved in swallowing and neck movement, so dose and injection depth are calculated conservatively. Treatment here is performed only after examination and is not suitable for every patient.

This treatment does not address sun damage, crepey skin texture or loss of volume in this area. If those are your main concerns, your doctor will discuss other options at the consultation rather than proceeding with injections.

The same injections used to relax facial muscles are also used, at different sites and doses, to reduce excessive sweating. This is a medical indication rather than a cosmetic one, and it is assessed by a doctor before anything is planned.

Where it is used

Underarms — the most common request and the area that responds most predictably. Palms — effective, but the injections are more uncomfortable and a nerve block is often used. Soles — possible, technically harder, and results are more variable. Scalp and forehead — helps where sweating interferes with work or social life.

How it works

The signal that tells the sweat glands to switch on is blocked locally at the injection sites. The glands themselves are not removed or damaged, and sweating elsewhere on the body is unaffected — the body does not compensate by sweating more somewhere else, which is the question patients ask most often.

What to expect

• The appointment takes 20 to 30 minutes, with anaesthetic cream beforehand for the underarms.
• The effect begins within a few days and is established at two weeks.
• It typically lasts four to six months in the underarms, often less on the palms.
• Normal activity the same day; avoid the gym, sauna and hot showers for 24 hours.

Before you book

Excessive sweating can have an underlying medical cause — thyroid disorders, medication, hormonal change, infection. A doctor takes a history and, where indicated, arranges tests before treating the symptom. Treating the sweating without asking why it started is not medicine.

In the Dubai climate the demand is seasonal for many patients: treatment is often planned in spring so the effect covers the hottest months.

The masseter is the muscle that closes the jaw. Where it is overactive or enlarged, two different problems follow, and the same injection addresses both.

Clenching and grinding

Night-time clenching wears the teeth, strains the jaw joint and causes morning headaches and jaw ache. Relaxing the muscle reduces the force of clenching. It does not cure the habit — a dental splint and, where relevant, management of the stress driving it remain part of the plan, and your dentist should be involved.

Jawline width

An enlarged masseter widens the lower face and squares the jaw outline. Reducing muscle bulk narrows it over six to eight weeks. The change is gradual and looks like weight loss in the face rather than a procedure.

What to expect

• Effect on clenching begins in one to two weeks; the visible slimming takes six to eight.
• Duration is typically four to six months, and repeat treatments often need less material as the muscle reduces.
• Chewing tough food may feel tiring for the first two weeks — that is expected and it settles.

Who it does not suit

Where the lower face is wide because of bone rather than muscle, this treatment changes nothing, and an honest examination will tell you which of the two you have. Where jaw pain comes from the joint rather than the muscle, the assessment belongs with a dentist or maxillofacial specialist first.

Before treatment

Every treatment begins with a consultation. The doctor reviews your medical history, current medication (including blood thinners and certain antibiotics), previous injectable treatments, pregnancy or breastfeeding status, and any neuromuscular condition. Your facial movement is assessed and usually photographed for your medical record.

During the appointment

Injections are made with a fine needle and the procedure itself usually takes ten to twenty minutes. Most patients describe brief stinging at each injection point. Topical anaesthetic cream or cooling can be applied beforehand if you prefer.

Afterwards

You can return to normal activity the same day. We ask you to stay upright for about four hours, avoid rubbing or massaging the treated area, and postpone strenuous exercise, saunas and facial treatments for 24 hours. Small red marks or bruising at the injection points are common and usually settle within a few days.

Onset and duration

The effect develops gradually rather than immediately — usually beginning within three to five days and settling by around two weeks. How long it lasts differs between patients and between treatment areas, and most people return for repeat treatment after several months. A review appointment two weeks after treatment is included so any adjustment can be made.

Risks and who should not have treatment

Like any injectable treatment this has risks, and your doctor explains them to you at the consultation, before you give consent. Treatment is not given during pregnancy or breastfeeding, where there is infection at the injection site, in known allergy to the product, or in certain neuromuscular disorders such as myasthenia gravis.
