“Liposuction” is not one operation. What changes the result is how the fat is loosened, how it is removed, and what is done about the skin afterwards. At ELEMENTAL three technologies are used, separately or together, and which combination suits you is decided at the consultation after the surgeon has examined the area and assessed your skin.

VASER — ultrasound-assisted liposuction

VASER stands for Vibration Amplification of Sound Energy at Resonance. Fine probes deliver ultrasound energy into the fat layer, and the energy breaks the fat cells apart into an emulsion before anything is aspirated. The frequency and the probe design are chosen so the energy acts mainly on fat, which is softer than the surrounding connective tissue, nerves and vessels.

The practical consequence: fibrous areas that resist conventional cannulas — the male chest, the back, the upper abdomen, areas of previous surgery — become workable, and the surgeon can remove fat closer to the skin surface where more defined contouring is required. Because the fat is emulsified first, less mechanical force is needed to extract it.

MicroAire — power-assisted liposuction (PAL)

A MicroAire cannula vibrates back and forth at high frequency along its own axis. That reciprocating movement does the mechanical work of passing through tissue, instead of the surgeon repeatedly forcing the cannula by hand.

Two things follow. Extraction is more even, because the cannula advances through fat in small controlled increments rather than in long forced strokes. And on larger volumes, or where scar tissue from previous liposuction has to be crossed, the operation is more controlled. PAL is frequently combined with VASER: ultrasound to emulsify, powered cannula to aspirate.

J-Plasma — helium plasma for skin retraction

Removing fat leaves the skin envelope that was covering it. In younger skin with good elasticity, it retracts on its own. Where elasticity is reduced, J-Plasma is applied under the skin after the fat is removed: helium plasma energy is delivered subdermally with the aim of contracting the fibrous layer.

It is important to be clear about the limit. J-Plasma addresses moderate laxity. It is not a replacement for skin excision: where there is a significant apron of loose skin or a stretched abdominal wall, the honest answer is abdominoplasty, and the surgeon will say so rather than sell a treatment that cannot deliver what you want.

How the choice is made

Not every patient needs all three. A young patient with firm skin and a localised deposit may need only power-assisted extraction. A patient with fibrous tissue and moderate laxity may need all three in one operation. The decision follows the examination — skin quality, fat character, the area involved and what you want changed — and it is explained to you with its reasoning before you consent. Individual results vary from patient to patient.

Areas differ in fat character, skin behaviour and how visible the result is, so they are planned differently rather than treated as one procedure applied in several places.

• Abdomen and flanks. The most commonly requested area and the one where skin quality matters most. Fibrous upper-abdominal fat responds to ultrasound emulsification; the flanks and waistline are what create the change in silhouette that patients actually notice in clothes.
• Arms. Upper-arm contouring is limited by skin elasticity more than by the volume of fat. Where the skin retracts, liposuction alone works well; where it does not, the surgeon will discuss whether skin removal is the realistic answer instead of promising retraction that will not happen.
• Submental area — the double chin and jawline. A small-volume, high-visibility area treated with fine cannulas, often with skin tightening, because even a modest change alters the jawline in photographs. It is one of the few areas where the difference is visible to the patient within weeks.
• Back and bra line. Dense fibrous fat that conventional cannulas struggle with, which is where ultrasound assistance is most useful.
• Thighs and knees. Inner thighs are prone to skin laxity and are approached conservatively; outer thighs and the area above the knee usually contour predictably.
• Male chest. Fatty tissue in the male chest is typically fibrous and often mixed with glandular tissue. Where the component is glandular rather than fatty, liposuction alone will not correct it and excision is required — this is established at the examination, not afterwards.

Several areas in one operation. Treating adjacent areas together usually produces a more coherent contour than treating them separately — abdomen and flanks are a common example. There are limits on the total volume that can safely be removed in a single session, and where your plan exceeds them the surgeon will stage the work rather than compress it. Safety limits are not negotiable, and they are explained to you with the plan.

Liposuction from AED 20,000 per area. The final price is confirmed in writing after the surgical consultation and depends on the number of areas treated.

The procedure is quoted after the surgical consultation. Liposuction is priced by the number and size of the areas treated, whether J-Plasma skin tightening is used, the type of anaesthesia and the length of the operation — and those are settled once the surgeon has examined you and you have agreed what is being treated.

After the consultation you receive a written estimate with a fixed price covering the operation, theatre, anaesthesia, the compression garment and post-operative reviews, so nothing is added later.

What liposuction is not. It is a contouring procedure for localised fat deposits that have not responded to diet and exercise — not a treatment for obesity and not a substitute for weight loss. It does not remove loose skin: where skin laxity is the main issue, the surgeon will say so and discuss whether abdominoplasty or skin-tightening is the more honest answer. Results are best judged once your weight is stable.
