Orthodontics moves teeth into a planned position and corrects the way the upper and lower teeth meet. At ELEMENTAL this is done for adults, with fixed appliances or removable aligners depending on what the teeth actually have to do.

Appearance is what brings most people to the consultation. The functional argument is usually the stronger one, and it is the one that decides whether treatment is worth doing at all: crowded teeth hold plaque where no brush reaches and cost their owner fillings and gum treatment for decades; an uneven bite loads a few teeth far harder than the rest and wears them down; a deep bite traumatises the gum behind the lower incisors; teeth that have drifted into the space of a lost molar make replacing that molar complicated and expensive.

Straightening teeth is therefore not only cosmetic dentistry. For an adult who intends to keep their own teeth, it is one of the cheaper forms of preventive dentistry available.

Two routes, and the choice follows the case rather than the look of the appliance.

Fixed braces are bonded to the teeth and work continuously, which makes them predictable for the movements that are hardest to control: closing extraction spaces bodily, uprighting tipped molars, correcting large rotations and managing bite discrepancies. Ceramic brackets make them considerably less visible than they used to be.

Clear aligners are a series of removable trays with small tooth-coloured attachments bonded to selected teeth. They are close to invisible, they come out for eating and cleaning, and they handle crowding, spacing and many mild to moderate bite problems well. Their one honest limitation is that they work only while they are worn — twenty to twenty-two hours a day is the treatment, not a suggestion.

Some cases are treated with aligners plus a short period of fixed appliances on a few teeth. That is a normal plan, not a failure of the first choice.

Orthodontics is planned on measurements, not on impressions of the eye. Records are clinical photographs, an intraoral scan of both arches, a panoramic radiograph and a lateral cephalometric radiograph, with a cone beam scan where the question is three-dimensional: an impacted canine and its relationship to the incisor roots, an unerupted or ectopic tooth, root proximity, how much bone covers the lower incisors, the position of third molars. Two conditions have to be met before any appliance is fitted. Active decay is treated, because a bracket bonded over decay hides it for two years. Gum inflammation is stabilised, because applying force to teeth whose support is inflamed accelerates bone loss — that is gum treatment, and it is not a formality.

Hygiene during treatment decides how the teeth look when the appliance comes off. Decalcification marks around brackets are permanent.

Teeth do not stay where they are put. The fibres around each root and the surrounding bone need months to reorganise, and teeth continue to drift slowly throughout life whether or not they were ever treated. Retention is what preserves the result, and it does not have an end date.

It is a thin wire bonded behind the front teeth, a removable retainer worn at night, or both. Bonded wires are checked at routine visits because they debond quietly; removable retainers are replaced when they wear or distort. Stopping because the teeth feel settled is the single most common reason people find them crowded again three years later — and a second course of treatment costs considerably more than a replacement retainer.

Ask what retention costs and whether it is inside the quoted fee before treatment starts. It is the line most often left out of an orthodontic quotation, here and everywhere else.

Most adult courses run between twelve and twenty-four months. Alignment of the front teeth alone can be shorter; cases involving bite correction, extraction space closure or impacted teeth run longer. The estimate is given after the records are read and, for aligners, after the digital setup is built — and it holds only if the appliance is actually worn.

A consultation starts from AED 650. Records and the course itself are quoted separately in writing. The estimate lists the stages: records and setup, the appliance, review appointments, refinements where the plan uses them, removal and polishing, and retention.

Orthodontics is also worth sequencing correctly against other work. Where veneers or crowns are planned afterwards, moving the teeth into position first usually means removing less tooth structure later — and less tooth removed is the one saving that compounds over a lifetime.
